Proven Health Strategies for Better Outcomes
The Center for Better Aging puts a complete healthcare ecosystem in one place. CBA recruits and trains providers to address the critical shortage of South Side providers with clinical expertise in the unique needs of aging adults.
CBA has partnered with St. Bernard Hospital to bring Chicago’s South Side a seamless and convenient healthcare experience. Enjoy access to primary care, urgent care, geriatric-certified ER (GEDA), diagnostics, and pharmacy services, all conveniently located on our state-of-the-art campus.
CBA member advocates work with providers to adopt best practices and technology for managing heart disease, diabetes, strokes, chronic illness, and mental and cognitive illness:
CBA Center
Determined effort can change the course of aging. CBA has recruited a primary care team skilled in preventive medicine. They consider family health history and support systems to assess your health risks, then work one-on-one with patients on care plans to avoid illness, manage chronic conditions, and maintain active lifestyles. Member advocates can give health providers a full picture of your care across the CBA system, so that doctors respect your concerns before, during, and after treatment.
Geriatric Emergency Department
Older hospital patients receive superior emergency care through programs such as the Geriatric Emergency Department Accreditation (GEDA). St. Bernard Hospital has the only South Side emergency department that earned the GEDA certification. This American College of Emergency Physicians program ensures the ER has the staffing and training to provide well-coordinated, quality care.
Specialty Services
South Side older adults often face long-distance visits to medical specialists. CBA maintains a network of community-based providers in and out of the Center. The campus has specialists for behavioral health, cardiology, chronic disease, diabetes, ear, nose and throat, gastroenterology, podiatry, nutritional counseling, orthopedics, rehabilitation, and pulmonology.
Behavioral Health
Isolation, grief and memory challenges often await in the aging journey. CBA looks after your behavioral health needs and gives you coping strategies to stay active in community and family life. The compassionate and qualified staff includes licensed clinical counselors, clinical social workers, and nurse practitioners certified in psychiatry.
St. Bernard Hospital Lab Services
Prescription and over-the-counter drugs can be essential to prevent and recover from illness, but multiple medications can interact with each other to produce negative side effects. A geriatric pharmacy program keeps a current list of medications and follows up with patients, so that they can take the right medicines at the right time to get the intended results without adverse drug interactions.
